This is a self-awareness tool, not a diagnostic instrument. If your scores consistently show low wellbeing, please reach out to a health professional, counsellor, or trusted friend. Self-care assessment is the beginning of help, not a substitute for it.

Preparation Notes

You will need:

  • 10-15 minutes of quiet reflection time
  • Honesty with yourself
  • A journal or the Libre Grow app
  • Optional: review your previous Self-Care Assessments for trends
Procedure Steps (Version 1.0)

Find a quiet moment. Rate your sleep quality over the past week — not just hours, but how rested you feel.

Rate your physical activity level. Include gardening, walking, exercise, and any movement that raises your heart rate.

Estimate the hours you spent in your garden or green space this week. Include observation time, not just work.

Record how many minutes of intentional mindfulness practice you did this week. Include meditation, body scans, and mindful observation.

Rate your social connection — do you feel part of a community? Have you had meaningful conversations this week?

Rate your overall sense of wellbeing. This is a gut feeling, not a calculation. Trust it.

Look at your ratings. Identify the single area with the lowest score — this is where to focus your self-care this week.

Commit to one specific, achievable action to improve that area. Record it as your intention for the coming week.
